Community First Services and Supports (CFSS) is a Minnesota DHS program that empowers people with disabilities and older adults to direct their own home care services. 
As your consultation services provider, Lero Wellness & Consulting will educate you about CFSS, support you in writing your service delivery plan, review your plan, and provide ongoing support as needed so you always feel informed and in control.

We help participants understand how CFSS works and what is available to them.
